                To support and encourage the use of Educational Breeder Tanks as a teaching tool,   Fuku-Bonsai offers a special $25 discount to teachers, schools, or those donating Educational Breeder Tanks to schools.  This is strictly an honor system and must include an email address for contact.  Fuku-Bonsai will be developing teaching aids including graphic training aids, and a teachers manual.  Teachers are invited to sharing their thoughts, recommendations, experiences, and lesson plans.

      REGULAR MAIL-IN ORDERS:  Print this form,  manually fill out, include with your order form, and mail with check to Fuku-Bonsai,  PO Box 6000, Kurtistown, Hawaii 97660.  

        PAYPAL MEMBERS:  Copy pertinent portions of this form and email to  sales@fukubonsai.com  with a notation that a PayPal Payment will be coming. Be sure to include all needed shipping information in the email as to purchaser, where to ship, address, and day phone number (required by FedEx).

      TO ORDER BY EMAIL WITH PAYMENTS BY CREDIT CARD THROUGH PAYPAL.
